Which works better?

The evidence leans toward tirzepatide for weight loss. In a large real-world analysis, people on tirzepatide were about three times more likely to reach 15% weight loss than those on semaglutide, and roughly 82% hit at least 5% loss within a year versus about 67% on semaglutide. In trials, tirzepatide produced around 20%+ body-weight reduction versus about 15% for semaglutide.

That doesn't make semaglutide a bad choice — it's highly effective, more widely available, comes in an oral form, and may be cheaper or better covered depending on your insurance.

Side effects

Both cause similar gastrointestinal side effects — nausea, diarrhea, constipation — usually worst early and easing over time. Some data suggest tirzepatide may be slightly better tolerated, but individual responses vary. Both carry rare but serious risks and require a prescription.
